2.1.6.1 Evaluating the Risk of Suicide
The following questionnaire can help you to determine the level of risk of suicidal behavior in a survivor.
Responses can be used to identify if a survivor is at risk of suicide and help to highlight specific risk behaviors.
Question A: Looks at the lifetime history of suicidal ideation and/or suicide attempts
Question B: Looks at the frequency of suicidal ideation over the past 12 months
Question C: Looks at the risk of a suicide attempt
Question D: Looks at the self-reported likelihood of suicidal behavior in the future
Ask the survivor to select one answer for each question and make note of their answer.
A. Have you ever thought about or attempted to kill yourself?
1 - Never
2 - It was just a brief, passing thought
3 - I have had a plan, at least once, to kill myself, but I did not try to do it
3 - I have had a plan, at least once, to kill myself, and I really wanted to die
4 - I have attempted to kill myself, but I did not want to die
5 - I have attempted to kill myself, and I really wanted to die
B. How often have you thought about killing yourself in the past year?
1 - Never
2 - Rarely (1 time)
3 - Sometimes (2 times)
4 - Often (3-4 times)
5 - Very often (5 or more times)
C. Have you ever told someone that you were going to commit suicide or that you might do it?
1 - No
2 - Yes, once, but I did not really want to die
3 - Yes, once, and I really wanted to die
4 - Yes, more than once, but I did not want to do it
5 - Yes, more than once, and I really wanted to do it
D. How likely is it that you will attempt suicide someday?
0 - Never
1 - No chance at all
2 - Rather unlikely
3 - Unlikely
4 - Likely
5 - Rather likely
6 - Very likely
Add up the number of points next to each answer that the survivor selected.
Based on the severity of the results, you may need to share this information with the health center.
