4.4.4 Patient Discharge

Determine to where the patient is to be discharged, and make appropriate arrangements for transportation.

Complete discharge instructions, and give both verbal and written instructions to the patient.

Work with the advocate to identify appropriate resources and referrals. Inform the patient concerning options available in their community.
